Job Opportunity: Home Health Registered Nurse

Location: Medford, New York
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Description

  • Provide and coordinate home nursing care according to physician orders and agency policies.
  • Directly supervise and evaluate Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
  • Conduct home visits with an average patient load of 5-8 patients per day.
  • Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
  • Must be willing to travel up to 75 miles from the home clinic, averaging 250-300 miles weekly.
  • Schedule is typically Monday to Friday, 8:00 AM to 5:00 PM, with weekend rotations every 3-4 weeks depending on staffing.
  • On-call duties occur approximately every 3-4 weeks, depending on staffing.
  • Use of electronic medical records systems HomeCare HomeBase (HCHB), Pointcare, and OASIS is required.

Required Qualifications

  • Valid Registered Nurse (RN) license in New York.
  • Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
  • Minimum of 1 year of recent experience in Home Health nursing, including experience with adult patients.
  • Hospice nursing experience.
  • Proficiency with HomeCare HomeBase (HCHB) electronic medical records system.
  • Current BLS certification issued by the American Heart Association or American Red Cross.

Work Environment & Expectations

  • Dress code: Scrubs.
  • Productivity expectation: Minimum 30 units per week.
  • Patient visits and documentation activities are assigned points; contractors typically handle 6-8 points daily.
  • No guaranteed hours; if patient census is low, floating to a nearby clinic may be arranged to maintain hours.
  • Local candidates are accepted under the same terms as travelers.

Compliance & Documentation Requirements

  • Must complete a comprehensive background check including 7 years of history, sex offender search, EPLS/GSA/SAM, HHS/OIG, and state-specific searches as applicable.
  • Fingerprinting required prior to start, with specific state requirements (e.g., NY, PA, FL, NC).
  • Education verification of nursing degree required.
  • Drug screening: 10-panel drug test required.
  • Health documents required prior to start and updated annually:
    • Physical exam within 12 months confirming good health and no communicable diseases.
    • Negative TB test or chest X-ray within 12 months.
    • Proof of immunizations or titers for Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella, and COVID-19 vaccination or declination.
    • Seasonal flu vaccination or declination documentation.
  • Must review and sign multiple compliance documents prior to start, including but not limited to:
    • Confidentiality forms, HIPAA and privacy policies, compliance training, code of conduct, and home health core policy acknowledgements.
  • Must have personal or auto insurance policy under candidate’s name or list candidate as a driver.

Additional Notes

  • Weekend and on-call requirements vary by location and will be discussed during the interview.
  • Some positions may follow a Baylor shift schedule (4x10s, Friday-Monday), but most are standard 5x8 schedules.
  • Two professional references and work history verification (at least two most recent employers or last 7 years) are required.

This position is ideal for experienced Home Health RNs who are comfortable with travel, electronic charting systems, and managing a diverse patient caseload in a home care setting.
