Registered Nurse – CVICU (Travel/Contract)
Location: Madison, Wisconsin
Shift: Day, 12-hour shifts (7:00 AM – 7:30 PM), 3 shifts per week (36 hours/week)
Contract Length: 13 weeks
Job Details
- Work schedule includes every other weekend rotation and some holidays.
- Contract options: 13 or 26 weeks (13 weeks minimum).
Required Qualifications
- Valid Registered Nurse (RN) license in Wisconsin.
- Minimum 1 year of CVICU experience within the last year.
- BLS (Basic Life Support) certification issued by the American Heart Association.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
Health and Immunization Requirements
- TB Testing: Negative TB test or chest X-ray within required timeframes; TB questionnaire required annually.
- Vaccinations:
- MMR (Measles, Mumps, Rubella): 2 doses or positive titers required; medical or religious exemptions accepted with proper documentation.
- Varicella: 2 doses or positive titer required; exemptions accepted with documentation.
- Hepatitis B: Completed vaccination series with positive titer or facility-specific declination form if declining.
- Flu Vaccine: Annual vaccination or approved medical/religious exemption required during flu season (Sept 22 – Apr 13).
- Tetanus/Diphtheria/Tdap: Proof of one adult Tdap vaccination required if working in pediatric areas.
- Meningococcal vaccine or declination required if working in Microbiology department.
- Drug Testing: Required for all clinical candidates per standard; pharmacy department candidates require specific drug screen within 45 days of start.
- Physical Exam: Must be collected within 1 year to meet agency standards (not facility required).
- Fit Mask Testing: Completed onsite; no copy required by agency.
Facility-Specific Requirements
- Photo for Badge: Passport-style photo required prior to start (no selfies, headwear except for religious/medical reasons, solid background).
- Communicable Disease Form: Must be completed within 90 days of start.
- Exemption Forms: Medical and religious exemption forms required for flu, MMR, and Varicella vaccines if applicable.
- Acknowledgments:
- Orientation Safety Manual Acknowledgement (annual).
- Pass/Fail Telemetry Testing Acknowledgement if applicable.
- Wisconsin State Mandated:
- BID Form and Caregiver Screening required within 90 days of start; both must be stored in compliance file.

- Prior travel experience is not explicitly required for this position.
- All certifications and licenses must be verified through primary source within 30 days of start and upon expiration.
