Description
Registered Nurse - MS/Tele Float (Travel/Contract)
Location: Madison, Wisconsin
Shift: Nights, 12-hour shifts (1900 - 0730), 3 shifts per week, every other weekend, holidays as applicable
Contract Length: 13 weeks
Job Description
- Work primarily on different Medical/Surgical units including cardiac, spine, and neuro specialties.
- Must pass a Telemetry test prior to starting.
- Experience with Epic electronic health record system is required.
Required Qualifications
- Valid RN license in Wisconsin.
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN).
- Telemetry experience within the last year.
- Minimum of 1 year of telemetry experience.
- Basic Life Support (BLS) certification from the American Heart Association (AHA) required.
- Experience with Epic electronic health record system is required.
Certifications & Testing
- BLS (AHA) required; renewals must be finalized by the 15th of the month prior to expiration.
- Telemetry testing (Adult Telemetry Test via LMS WorkRamp) required 2 Fridays prior to start and annually thereafter.
- Must score at least 80% overall and 100% in lethal rhythms subcategory.
- Up to 3 attempts allowed; remediation required after 2nd failed attempt. Failure after 3rd attempt results in contract cancellation.
- Orientation Safety Manual Acknowledgement required annually.
Compliance & Background Checks
- Background check within 90 days prior to start, including SSN trace, counties, and nationwide sex offender search.
- Wisconsin Caregiver Screening and BID Form required within 90 days of start. Both must be stored in compliance file.
- EPLS/GSA/SAM and HHS/OIG checks required upon hire and monitored monthly thereafter.
- Education verification as applicable (for RN, standard nursing education verification applies).
- Drug screening required per Vaya standard (clinical candidates).
Health & Immunization Requirements
- Negative TB test or chest X-ray within required timeframes; TB questionnaire required annually.
- Proof of MMR (Measles, Mumps, Rubella) immunity or valid medical/religious exemption.
- Varicella immunity proof or valid exemption.
- Hepatitis B vaccination series completion or valid declination form if declining.
- Seasonal flu vaccination or approved medical/religious exemption required annually during flu season (09/22 – 04/13).
- Physical exam must be collected to meet Aya standard (within 1 year).
- Fit mask completed onsite (no copy required).
Additional Requirements
- Passport-style photo for badge (no selfies, no headwear except for religious/medical reasons).
- References: 2 clinical references within the last 3 years (Aya standard).
- Employment application and annual skills checklist required.
