RN Case Manager
Location
- Tuba City, Arizona

Schedule
- Monday to Friday, 8am–5pm
- Callback on weekends/holidays
Role Summary
- Outpatient/Inpatient Nurse Case Manager supporting high-risk patients
- Focuses on care transitions, discharge planning, care coordination, and utilization review
- Works with culturally diverse patients across all ages, including patients with mental health needs
Required Qualifications
- Associate of Science in Nursing / Associate Degree in Nursing
- Valid RN license in Arizona
- Case Management (3 years of experience)
- Case Management experience within the last year
- Experience in a(n) Outpatient setting
- Experience in a(n) Inpatient setting
- BLS issued by American Heart Association
- Utilization Review (InterQual)
Additional Requirements
- Fingerprinting must be submitted and cleared before start
Housing
- Shared contractor housing only
- Very limited hospital rental property
- No pets or family allowed per policy
