Discharge Planner
Overview
- Contract role in Fort Defiance, Arizona.
- Schedule: Monday through Friday, 8:00 a.m. to 5:00 p.m. with one-hour lunch.
- Assignment length: 13 weeks.
- Housing is limited and available at additional cost.
Role Details
- Performs case management and coordinates discharge planning for age-appropriate in-patients with chronic diseases and special health care needs.
- Based in Multi-Service Unit, Intensive Care Unit, OB Units, and Adolescent Care Unit.
- Travelers are required to attend staff meetings when scheduled.
- Travelers are required to attend New Employee Orientation, scheduled every other Monday.
- Travelers must be cleared 7 days prior to the start date.
Required Qualifications
- Valid RN license in Arizona
- Associate of Science in Nursing / Associate Degree in Nursing
- 2 years of Med Surg experience within the last year
- Experience in an Acute Care or Critical Care setting
- Case Management experience
- BLS issued by American Heart Association
