Opportunity:

RN - Case Management

Job Details:

• Work 8-hour day shifts from 8:00 AM to 4:30 PM with potential for flexible start and end times
• 40 guaranteed hours per week over 13-week contract
• Every other weekend required with holiday rotation
• On-call responsibilities 1-2 times during the week and every other weekend
• Must respond to answering service calls within 15 minutes
• Call back minimum of 2 hours when called in
• Travel to see 20-25 patients with average of 4.5 visits per day
• Daily travel can vary from 50-100 miles depending on territory and patient census
• Mileage reimbursed at government approved rate
• May float to other territories to assist when needed during low census
• Can be placed on call for low census hours
• Required certifications: Wisconsin license, AH ABLS, valid driver's license
• OASIS certification is a plus
• Minimum 2 years of home health experience required
• Travel experience required
• EPIC and OASIS charting experience required
• Perform OASIS admissions, all time points, episode management, doctor calls and care coordination
• Clinical competencies in wound care, IV therapy, ports, lab draws, and chronic disease management
• Mask required at all times during flu season if flu and COVID vaccinations are declined
