Opportunity:

RN - Case Management

Job Details:

• Work Monday through Friday from 8:00 AM to 4:30 PM
• Home hospice position visiting patients in their homes
• Manage caseload of 12-15 patients
• Coverage territory includes Lynwood, Calumet, Calumet City, Calumet Park, Riverdale, and Dolton
• No weekend, on-call, or holiday responsibilities
• Set schedule with fixed shift times
• 1-2 week orientation based on comfort level and experience
• Required certifications: BLS, valid driver's license, auto insurance, and personal vehicle
• 3 years of home hospice or home health experience required
• Accept first-time travelers
• Scrub color: Navy blue or any solid professional color (no graphics, logos, or designs)
• Free parking available
• Provided equipment includes phone, laptop, and trunk stock
• EMR system: EPIC Remote Client
• Skills required: wound care, dressing changes, wound vacs, drain care, care coordination, end of life care/planning, pain/symptom management, triage, and patient/family education
• Strong organizational and time management skills essential
• Other hospice RNs and management available via phone for support
