Opportunity:

RN - Case Management

Job Details:

• Work Monday through Friday from 8:00 AM to 5:00 PM with flexible scheduling options including Tuesday through Saturday when needed
• Must be available by phone during scheduled hours
• Attend daily huddle at 7:50 AM via phone or Teams if traveling to see patients
• Manage caseload of 22-25 patients per week with approximately 6 visits per day
• Provide remote field work visiting patients in their homes, family member homes, assisted living facilities, group homes, and nursing homes
• Care for high acuity patients including pleurx drains, PCA pumps, IV antibiotics, TPN, Foleys, wounds, wound VACs, chest tubes, and various drains
• Perform patient and family education, provider communication, and disease management
• Troubleshoot and demonstrate knowledge of pleurx drains, nephrostomy tubes, trachs, PCAs, and gastric suctioning
• Required certification: BLS
• EPIC EMR experience preferred
• No weekend rotation, call responsibilities, or holiday expectations
• Up to 2 weeks orientation based on comfort level
• Any color scrubs acceptable
• Laptop, cell phone, and PPE provided
• Must complete N95 fit testing prior to arrival
• Minimum 1 year experience required; first-time travelers accepted with 1 year home health experience
