Opportunity:

RN - Case Management

Job Details:

• Work Monday through Friday from 8:00 AM to 4:30 PM
• Provide home health services to patients throughout the Fox Valley and Oshkosh areas
• Manage caseloads consisting of patients in specialty areas with average census of 165 patients and 6 units daily
• Weekend backup rotation with other nurses approximately 1-5 weekends
• On-call responsibilities 1 night per week and alternating weekends
• Conduct initial assessments and establish plans of care according to physician orders
• Provide direct patient care and ongoing assessments with modifications as needed
• Complete documentation including visit assessments, OASIS, 485 forms, and verbal orders
• Communicate with physicians and healthcare team members for care coordination
• Facilitate discharge planning and ensure continuity of care
• Care for patients with wound care needs, COPD, CHF, diabetes, total joints, and medication management
• Administer IV medications and manage PICC lines
• Must have valid driver's license and personal vehicle for travel between patient homes
• Minimum 1 year of nursing experience required
• First-time travelers accepted
• Wisconsin RN license required
• BLS certification required within 6 months
• Associate's Degree in Nursing required
• Home health experience and Epic EMR experience preferred
• One week orientation provided
• No specific scrub color requirements

The organization utilizes modern technology including Epic EMR system and provides nurses with necessary equipment such as home care bags, medical supplies, laptops, and mobile phones with hotspot capabilities for remote documentation and communication.
