Opportunity:

RN - Case Management

Job Details:

• Work daytime hours with flexible start and end times based on patient visits
• Productivity-based model requiring typically 5-6 home visits per day depending on patient acuity and geography
• No weekend rotation, on-call duties, or holiday requirements
• Weekly scheduling cycle with case-by-case RTO approvals
• Travel within designated North 4 geographic coverage area and may support neighboring teams as needed
• 2+ years of nursing experience preferred with home health experience strongly preferred
• Will accept first-time travelers with strong relevant experience and adaptability
• Active Wisconsin RN license and BLS certification required
• Valid driver's license and reliable transportation required
• Associate or Bachelor's degree in Nursing required
• Strong clinical assessment skills, wound care, and medication management abilities required
• OASIS documentation experience preferred
• Must be able to work independently in the field
• Professional attire expected with no strict scrub color requirements
• Standard home health orientation lasting 5-7 days including Epic review and field precepting
• Use Epic EMR system and standard home health equipment including wound care supplies and portable devices
