Opportunity:

Physical Therapist - Physical Therapy

Job Details:

• 13-week contract with an ASAP start date
• 8 variable day shifts Monday through Friday
• Guaranteed 38 hours per week with flex up to 40 hours per week
• No holiday rotation required
• Physical therapists rotate one Saturday every 6 to 8 weeks with a compensatory day off during that same week
• May be placed on low call during low census periods with a minimum 2-hour call back
• Occasional travel to central and northern regions required
• Mileage reimbursed at government approved rate
• Evaluate, treat, and plan patient care in accordance with Physical Therapy standards of practice
• Serve adult and geriatric patient populations with a variety of orthopedic, neurological, and functional mobility diagnoses
• Meet productivity expectations of an average of 4.5 visits per day
• Document to meet regulatory and reimbursement requirements using NetSmart EMR
• Use provided laptop for point of service documentation and cell phone for GPS and communication
• Scrub colors: any or business casual attire
• Required certifications: AHA or ARC BLS, valid Wisconsin state licensure, valid driver's license with proof of car insurance, and NPI number required
• Required education: Bachelor's Degree in Physical Therapy with knowledge of Physical Therapy standards and practices acquired through a minimum of a Master's Degree in Physical Therapy
• Minimum experience: Home health experience with training working with adult and geriatric populations required
• Contractors must live outside a 50-mile radius of the primary location
