Opportunity:

RN - Case Management

Job Details:

• Work Monday through Friday, 8:00 AM to 4:30 PM with occasional late stays, plus every 5th weekend rotation
• When working a weekend, will have a Monday and Friday off following that weekend
• Set schedule with weekend rotation
• No on-call responsibilities
• Holiday expectations include 50% or 2 holidays during contract
• Manage a caseload of approximately 14 to 20 patients
• Work with an underserved population, primarily adults and geriatrics, including complex cases involving unhoused and low-income patients
• Assigned to a specific unit with possibility of floating to other inpatient units such as PCU, MedSurg, or LDRP
• Collaborate with multidisciplinary team including physicians, hospitalists, other case managers, and case management assistants
• Navigate various insurance plans and assist with facility initiatives
• Utilize strong assessment, critical thinking, and interpersonal skills to find resources for patients
• Must be a team player and comfortable asking questions and advocating for patients
• Use EPIC EMR system and Careport for referrals
• Minimum 3 years of acute care case management experience required
• Associate of Science in Nursing (ASN) or higher accepted
• Required dress code: Eggplant colored scrubs or business casual attire with white lab coat required

This facility is a 205-bed inpatient hospital featuring a Level II NICU and serves a diverse patient population with a focus on underserved communities. The hospital supports its case management team with robust resources including leadership, hospitalist coverage available 24/7, case management assistants, and access to community programs and referral tools.
