Description
Job Summary:
Long Term Care (LTC) Registered Nurse (RN) Miles City, Montana. The RN in Extended Care utilizes the nursing process (assessment, diagnosis, planning, implementation, and evaluation) to deliver holistic, age-specific care. This role includes resident advocacy, coordinating total care, maintaining quality standards, and supporting documentation and compliance requirements. Candidates are required to bring their license on the first day.
Job Details:
- Schedule: Days (12 hours per shift); minimum 36 hours per week
- Required Qualifications: Current Registered Nurse license in the state of practice; Basic Life Support (BLS) American Heart Association; ASN (Associate of Science in Nursing) graduation from an accredited School of Nursing; ability to read and write at a level to fulfill documentation requirements

- Responsibilities:
- Completes holistic, age-specific admission history, physical assessment, and nursing plan of care on assigned residents to monitor for deviations
- Analyzes and incorporates clinical data (e.g., lab, x-ray, and other diagnostics) to formulate an individualized plan of care
- Identifies and documents residents at risk for nutritional and functional status
- Identifies and reports signs and symptoms of neglect or abuse related to child, domestic, or elder abuse
- Sets priorities for and reviews daily plan of care in collaboration with resident, family/significant other, and other healthcare disciplines
- Delegates tasks based on resident needs, continuity of care, staffing, and team knowledge
- Communicates with physician(s) to coordinate the medical and nursing plan of care
- Demonstrates competence with technical skills and proactively manages urgent or emergent resident care situations
- Prepares for and administers medications and IVs per scope of practice and facility policy
- Documents nursing interventions, resident responses to care, and resident outcomes; revises the plan of care based on changing needs
- Provides resident/family education based on assessment, coordinates education and discharge planning needs, and supports discharge activities and required documentation
- Maintains working knowledge of CMS regulations and Medicare skilled and MDS criteria to support documentation compliance and reflect resident status
- Supervises, coaches, counsels, and evaluates assigned C.N.A. staff
