Description
Job Summary:
Long Term Care Registered Nurse (RN) in Miles City, Montana. This position involves providing comprehensive nursing care to residents in a long-term care setting. The RN will utilize the nursing processincluding assessment, diagnosis, planning, implementation, and evaluationto ensure high-quality, resident-centered care and promote optimal functional and health outcomes.
Job Details:
- Completed holistic, age-specific admission history, physical assessment, and nursing care plan on all assigned residents, with ongoing monitoring for deviations.
- Analyzed and incorporated clinical data such as lab results, x-rays, and diagnostics to formulate individualized care plans.
- Identified and documented residents at risk for nutritional and functional decline.
- Recognized and reported signs/symptoms of neglect/abuse related to child, domestic, or elder abuse.
- Sets priorities and reviews daily plans of care in collaboration with residents, families, and healthcare team members.
- Delegates tasks to team members based on resident needs, staff capabilities, and continuity of care.
- Communicates with physicians to coordinate medical and nursing care plans.
- Demonstrates technical competence in performing nursing skills and managing urgent/emergent resident care situations.
- Administers medications and IV therapy in accordance with scope of practice and facility policies.
- Documents nursing interventions, resident responses, and outcomes, and evaluates resident progress to revise care plans accordingly.
- Provides resident and family education based on assessed needs, including discharge planning and instructions related to medications, wound care, and follow-up.
- Maintains knowledge of CMS regulations and Medicare skilled criteria to ensure documentation compliance supporting resident care needs.
- Incorporates resident preferences into organized, efficient care delivery, delegating tasks appropriately.
- Supervises and evaluates performance of Certified Nursing Assistants (CNAs) and other team members.
- Schedule: Night shift, 12 hours per shift, minimum of 36 hours per week.
- Certifications & Licenses: Valid RN license in the state of practice and current BLS certification (American Heart Association).
- Education: Graduation from an accredited School of Nursing (ASN).
