Contract | Days | Full-Time | Onsite | Monday–Friday with flexible scheduling options available (e.g., Tuesday–Saturday, Sunday–Thursday, or 4x10-hour shifts)

Key Responsibilities

• Assess clients' needs and develop individualized therapy plans; administer and perform therapeutic interventions in SNF and Home Health settings
• Document observations, services provided, and clinical notes using Net Health and HCHB software
• Notify physicians of changes in client condition and process orders; collaborate with interdisciplinary team members
• Provide training on programs, adaptive equipment, and other therapeutic interventions
• Supervise therapy assistants in accordance with Federal and State regulations and document supervision appropriately in the clinical record
• Follow infection control, safety standards, and bag techniques; maintain client privacy, dignity, and confidentiality

Required Qualifications

• Completion of an accredited physical therapy program required
• National registration and licensure for the state of practice (Minnesota)
• Must be at least 18 years of age
• Ability to communicate effectively in English, both orally and in writing
• Ability to meet SNF productivity expectation of 82% and Home Health productivity expectation of 90% (inclusive of drive time and documentation)

Preferred Experience / Skills

• Prior work experience as a Physical Therapist in a SNF or Home Health clinical setting
• Experience with Net Health and/or HCHB documentation platforms
• Experience supervising Physical Therapy Assistants
