Physical Therapist (PT)

Key Responsibilities

Evaluate and treat patients requiring physical therapy services in a skilled nursing or long-term care setting
Develop and implement individualized treatment plans based on patient needs and goals
Monitor and document patient progress, adjusting treatment plans as appropriate
Collaborate with interdisciplinary care teams to ensure comprehensive patient care
Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards

Required Qualifications

Active Physical Therapist (PT) license in the state of Nebraska or eligibility to obtain
Graduate of an accredited Physical Therapy program
Current CPR/BLS certification
Ability to commit to a guaranteed weekly schedule of 36–40 hours depending on shift length

Preferred Experience / Skills

Prior experience in a skilled nursing facility (SNF) or long-term care environment
Familiarity with geriatric patient populations and related rehabilitation needs
