Physical Therapist – Home Health
Location
Seaside, OR

Contract | Full-Time | Day Shift (5x8 Hours, 8:00–16:30) | Onsite / Field-Based
Key Responsibilities
- Provide skilled physical therapy services to home health patients in Seaside, OR and surrounding areas
- Conduct patient evaluations and deliver individualized treatment plans in a home-based setting
- Complete OASIS assessments accurately and in a timely manner
- Document patient care using EPIC Remote Client for Home Health and the Rover mobile application
- Visit up to 6 patients per day while meeting standard productivity expectations
- Travel to patient homes within a defined service area, with occasional drive times of up to 60–90 minutes per day
Required Qualifications
- Minimum 1 year of skilled home health physical therapy experience
- Active or pending Physical Therapist license in the state of Oregon
- Basic Cardiac Life Support (BCLS) certification required
- Proficiency with OASIS documentation
- COVID-19 primary vaccination series plus at least one booster required
- Must have reliable personal transportation
Preferred Experience / Skills
- Experience with EPIC Remote Client for Home Health and/or Rover application
- Familiarity with productivity point-based systems in a home health environment
- Comfort working independently across a broad geographic service area
