Key Responsibilities

Evaluate and treat patients in a Skilled Nursing Facility (SNF) setting
Develop and implement individualized physical therapy treatment plans
Monitor and document patient progress toward established therapy goals
Collaborate with interdisciplinary care team to coordinate patient care
Educate patients and caregivers on therapeutic exercises and home programs

Required Qualifications

Graduate of an accredited Physical Therapy program
Current and valid Physical Therapist (PT) licensure in the state of Missouri
Experience working in a Skilled Nursing Facility (SNF) setting
Current CPR/BLS certification

Preferred Experience / Skills

Prior travel or contract therapy experience
Familiarity with SNF documentation and Medicare compliance requirements
