Key Responsibilities

• Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
• Conduct 60-minute evaluation and treatment sessions in an outpatient setting
• Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
• Design functional treatment programs that support effective return to community activities and progress patients toward discharge within an established length of stay
• Serve as an educator for patients, families, students, and coworkers; participate in student programs as assigned by providing supervision, developing learning objectives, and completing student evaluations
• Adhere to discipline-specific State Practice Act and regulatory requirements; actively pursue professional development through continuing education and professional organizations

Required Qualifications

• Active and unrestricted Physical Therapist license in the state of California
• Outpatient physical therapy experience required
• Experience performing 60-minute evaluations and treatments
• Not eligible for new graduate candidates — prior clinical experience is required
• Knowledge of and adherence to state Practice Act and applicable professional codes of conduct

Preferred Experience / Skills

• Experience with functional quality indicator assessments and letters of medical necessity
• Demonstrated involvement in performance improvement initiatives or professional organizations
• Experience supervising or mentoring physical therapy students
