Contract | 13-Week Assignment | Onsite | Skilled Nursing Facility (SNF)

Key Responsibilities

Evaluate patients and develop individualized physical therapy treatment plans within a skilled nursing facility setting
Provide direct therapeutic interventions to improve patient mobility, strength, and functional independence
Monitor and document patient progress, adjusting treatment plans as clinically appropriate
Collaborate with interdisciplinary care teams to ensure coordinated and effective patient care
Educate patients and their families on therapeutic exercises and home safety strategies

Required Qualifications

Active Physical Therapist (PT) license in the state of Virginia or compact licensure eligibility
Graduate of an accredited Physical Therapy program
Current CPR/BLS certification
Experience working in a skilled nursing facility (SNF) or similar post-acute care environment

Preferred Experience / Skills

Prior experience in a SNF or long-term care setting
Familiarity with Medicare documentation and compliance requirements
