Physical Therapist (PT) - Home Health

Location

Torrance, CA

Schedule / Job Type

Contract | Days | Monday through Friday, 8:00 AM to 5:00 PM | 13-Week Assignment

Key Responsibilities

Provide home health physical therapy services across an assigned territory spanning Culver City south to Long Beach
Conduct Start of Care (SOC) evaluations and follow-up visits in the patient's home setting
Meet a daily productivity expectation of 6 points, which may consist of follow-up visits, Start of Care visits, or a combination thereof
Document patient care accurately and in a timely manner using the HCHB electronic medical records system
Develop and implement individualized physical therapy treatment plans in accordance with physician orders and patient goals

Required Qualifications

Active Physical Therapist (PT) license in the state of California
Experience with HCHB (Homecare Homebase) EMR system required
Prior home health physical therapy experience
Ability to manage an independent caseload across a defined geographic territory

Preferred Experience / Skills

Familiarity with the Culver City to Long Beach service area
Strong time management and self-directed work skills suited for a home health environment
