Key Responsibilities

• Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in alignment with patient needs, goals, and physician direction
• Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
• Ensure care is provided under signed, valid physician orders and within insurance approvals as required for patient clinical care
• Delegate and direct discipline-specific therapy services for assigned patients, designing functional treatment programs to support effective return to community activities
• Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations
• Actively pursue professional development through continuing education, self-study, and involvement in professional activities and organizations

Required Qualifications

• Active and unrestricted Physical Therapist license in good standing in the applicable state
• Minimum of 1 year of clinical experience as a Physical Therapist
• Adherence to discipline-specific State Practice Act and all applicable regulatory agency requirements
• Completion of all state-mandated continuing education requirements
• Demonstrated ability to perform in an ethical manner in accordance with the discipline-specific professional code of conduct

Preferred Experience / Skills

• Prior experience in an outpatient physical therapy setting
• Familiarity with performance improvement methodologies and quality of care initiatives
• Experience supervising or mentoring physical therapy students
