Job Title

Physical Therapist (PT) - Home Health

Location

Tukwila, WA

Schedule / Job Type

Contract | Full-Time | Days | Monday–Friday, 8:00 AM–5:00 PM | 13-Week Assignment

Key Responsibilities

Provide skilled physical therapy services to patients in a home health setting
Meet productivity expectations of 30 points per 40-hour work week
Manage a patient caseload primarily within a central and mid-county territory
Support additional service areas within the office radius as needed
Document all patient care accurately and timely using HCHB electronic medical records

Required Qualifications

Active and unrestricted Physical Therapist (PT) license in the state of Washington
Minimum of 1 year of home health experience with a Medicare-certified facility
Proficiency with HCHB (HomeCare HomeBase) EMR system required
Ability to work a Monday–Friday daytime schedule with no on-call or weekend requirements

Preferred Experience / Skills

Experience managing a multi-area home health territory
Strong time management and self-directed work skills in a field-based environment
