Job Title

Physical Therapist (PT) - Home Health

Location

Spokane Valley, WA

Schedule / Job Type

Contract (13 Weeks) | Full-Time | Onsite | Monday through Friday, 8:00 AM - 5:00 PM

Key Responsibilities

Provide skilled physical therapy services to patients in a home health setting
Evaluate and treat patients within an assigned territory covering Spokane County and potentially lower Stevens County
Develop and implement individualized treatment plans based on patient needs and goals
Document patient progress and clinical notes using HCHB electronic medical records
Collaborate with interdisciplinary care teams to coordinate patient care

Required Qualifications

Active Physical Therapist (PT) license in the state of Washington
Minimum of 12 months of Home Health experience within the past 3 years
Proficiency with HCHB electronic medical records system
Valid driver's license and reliable transportation for patient home visits

Preferred Experience / Skills

Familiarity with Spokane County and surrounding areas
Strong organizational and time management skills for managing a home health caseload
