Job Title

Physical Therapist (PT) - Home Health

Location

Anchorage, AK

Schedule / Job Type

Contract | Full-Time | Onsite | Monday–Friday, 8:00 AM – 5:00 PM | 13-Week Assignment

Key Responsibilities

Provide skilled physical therapy services to patients within the Anchorage municipality territory
Conduct patient evaluations, develop individualized treatment plans, and deliver therapeutic interventions in the home health setting
Complete OASIS assessments accurately and in accordance with regulatory requirements
Maintain productivity standards within a 40-hour work week
Document all patient care activities and progress in the electronic medical records system

Required Qualifications

Active Physical Therapist (PT) license in good standing
Minimum of 1 year of home health experience
Minimum of 1 year of OASIS experience
Ability to manage a full-time caseload with a 40-hour guaranteed work week

Preferred Experience / Skills

Experience with HCHB (Homecare Homebase) electronic medical records system
Familiarity with home health productivity and point-based documentation systems
