Job Title

Physical Therapist (PT) – Home Health

Location

Mission Viejo, CA

Schedule / Job Type

Key Responsibilities

Provide skilled physical therapy services to patients in a home health setting across an assigned territory
Evaluate, plan, and implement individualized treatment plans to meet patient goals and functional needs
Meet a productivity standard of 30 points per week
Document all patient care accurately and in a timely manner using the HCHB electronic medical records system
Participate in on-call and weekend coverage as required
Collaborate with the interdisciplinary care team to ensure coordinated and effective patient outcomes

Required Qualifications

Active Physical Therapist (PT) license in the state of California
Minimum of 1 year of home health experience with a Medicare-certified facility
Proficiency with HCHB (Homecare Homebase) EMR system required
Ability to meet weekly productivity expectations of 30 points per week
Availability to work Sunday–Thursday and participate in on-call and weekend requirements

Preferred Experience / Skills

Familiarity with the service territory including Capistrano Beach, Rancho Mission Viejo, Ladera Ranch, San Clemente, and San Juan Capistrano
Strong time management and organizational skills suited for an independent, field-based work environment
