Schedule / Job Type

Key Responsibilities

Provide outpatient physical therapy evaluation and treatment services to patients
Develop and implement individualized treatment plans based on patient needs and goals
Document patient progress and maintain accurate records using required charting systems
Collaborate with interdisciplinary team members to coordinate patient care
Float to other units or locations as needed based on facility demands

Required Qualifications

Active Virginia Physical Therapy license (must be in hand at time of submittal) or valid compact license with proof provided
Minimum one year of specialty experience within the past year
BLS certification through the American Heart Association (AHA)
One supervisory reference required at time of submittal
Social Security Number required at time of submittal

Preferred Experience / Skills

Experience in an outpatient physical therapy setting
Familiarity with EPIC electronic health records system
