Key Responsibilities

• Evaluate posture, gait, balance, coordination, and strength to develop individualized treatment plans aimed at restoring mobility and independence
• Lead, guide, and direct the delivery of physical therapy services within the facility in accordance with local, state, and federal regulations
• Implement treatment interventions to improve patient independence with functional tasks such as transfers, ambulation, and stair climbing
• Ensure all therapy services align with established facility policies, procedures, and long-term care standards
• Maintain accurate and timely clinical documentation in compliance with skilled nursing facility requirements

Required Qualifications

• Valid, unrestricted Physical Therapist license in the state of Missouri
• Meets all educational requirements associated with PT licensure
• Minimum of 1 year of physical therapy experience
• Valid driver's license in the state of employment
• Knowledge of skilled nursing home regulations and guidelines pertaining to long-term care

Preferred Experience / Skills

• Prior experience in a long-term care (LTC) or rehabilitation setting
• Familiarity with Centers for Medicare & Medicaid Services (CMS) standards and compliance requirements
