Job Title

Physical Therapist – Home Health

Location

Warsaw, VA

Schedule / Job Type

Contract | Days | Full-Time | 5x8-Hour Shifts (8:00 AM – 4:30 PM) | Onsite

Key Responsibilities

Provide physical therapy evaluations and treatments to patients in a home health setting
Complete and maintain accurate OASIS documentation in accordance with home health requirements
Develop and implement individualized patient care plans based on assessment findings
Monitor and document patient progress, adjusting treatment plans as appropriate
Collaborate with interdisciplinary care team to ensure coordinated patient outcomes

Required Qualifications

Active Physical Therapist license valid in Virginia (in hand at time of submittal or proof of compact license)
Minimum 1 year of experience in a home health setting
Proficiency in OASIS documentation
BLS certification through the American Heart Association (AHA)
One supervisory reference required at time of submittal

Preferred Experience / Skills

Experience with home health-specific electronic medical record systems
Strong communication and patient education skills in a home-based care environment
