Key Responsibilities

• Screen, evaluate, and rehabilitate patients with physical functioning disorders and/or wound care needs, developing appropriate plans of care in accordance with regulatory and clinical practice standards
• Utilize standardized assessments and evidence-based practice to guide clinical interventions including manual techniques, therapeutic exercises, ambulation, modalities, assistive devices, and wound care
• Monitor and document patient progress, adjust treatment plans accordingly, and evaluate treatment outcomes in the electronic documentation system per regulatory requirements
• Provide consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders
• Participate in required meetings such as patient care conferences, utilization review meetings, and rehabilitation meetings, as well as required trainings and in-services
• Supervise and support physical therapist assistants, rehabilitation aides, and/or students as applicable

Required Qualifications

• Active Missouri State Physical Therapist license
• Minimum of 1 year of physical therapy experience, or new graduate with completed Level II clinical experience
• Long Term Care (LTC) or rehabilitation setting experience
• Adherence to APTA and Missouri state practice acts, code of conduct, and professional ethics

Preferred Experience / Skills

• Experience with electronic documentation systems in a clinical setting
• Familiarity with DME procurement and wound care protocols within a long term care environment
