Job Title

Physical Therapist (PT) – Home Health

Location

Twin Falls, ID

Schedule / Job Type

Contract | Full-Time | Days | Monday–Friday, 8:30 AM–5:00 PM | Onsite / Field-Based (Home Health)

Key Responsibilities

Provide in-home physical therapy services to a caseload of 4–6 patients per day across a defined service area
Perform patient admissions, ongoing treatment of existing patients, and recertification evaluations
Treat a multidisciplinary adult patient population including elderly, post-CVA, post-surgical, orthopedic, and wound care cases
Participate in multidisciplinary team meetings held on Mondays and Wednesdays to discuss current admissions
Document patient care using Epic EMR with a remote client offshoot
Begin shift directly from home; travel throughout the assigned service area (up to approximately 60 miles east and west, 60 miles north, and 40 miles south)

Required Qualifications

Active Physical Therapist (PT) license in good standing
Minimum of 1 year of post-graduate PT experience
Prior home health experience required
Orthopedic experience required
Basic Life Support (BLS) certification through the American Heart Association (AHA)

Preferred Experience / Skills

Post-graduate home health experience strongly preferred
Experience with Medicare patient populations and rehab-focused care settings
Familiarity with Epic EMR
