Physical Therapist

Contract | Days | 36 Hours Guaranteed per Week

Key Responsibilities

Evaluate patients and develop individualized physical therapy treatment plans
Administer therapeutic exercises, manual therapy, and other physical therapy interventions
Monitor and document patient progress throughout the course of treatment
Collaborate with interdisciplinary care team to ensure coordinated patient care
Educate patients and families on home exercise programs and safe functional mobility

Required Qualifications

Active Physical Therapist (PT) license in the state of Wyoming or ability to obtain prior to start
Graduate of an accredited Physical Therapy program
Current CPR/BLS certification
Experience working in a skilled nursing or rehabilitation care setting

Preferred Experience / Skills

Prior experience in a long-term care or post-acute rehabilitation environment
Familiarity with Medicare documentation and compliance requirements
Strong communication and patient-centered care skills
