Key Responsibilities

Provide direct physical therapy services to patients in an outpatient or clinic-based setting
Evaluate, assess, and develop individualized treatment plans for patients
Deliver hands-on therapeutic interventions in alignment with plan of care goals
Complete required clinical documentation accurately and in a timely manner
Collaborate with the care team to ensure continuity and quality of patient care

Required Qualifications

Active Physical Therapist license in the state of Illinois (permanent license required; compact license not accepted)
Prior experience in a physical therapy clinical setting
Ability to meet credentialing and contract quality assurance requirements prior to assignment start

Preferred Experience / Skills

Experience working in an outpatient physical therapy environment
Familiarity with documentation systems used in clinical rehabilitation settings
