Key Responsibilities
- Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans based on patient needs, goals, and physician direction
- Conduct 60-minute initial evaluations and 30-minute follow-up sessions in an outpatient setting
- Complete accurate and timely medical record documentation including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
- Delegate and direct discipline-specific therapy services for assigned patients, ensuring care is provided under valid physician orders and within insurance approvals
- Serve as an educator for patients, families, students, and coworkers; participate in student programs as assigned, including supervision and completion of student evaluations
- Adhere to the applicable State Practice Act and regulatory requirements, and actively pursue continuing education and professional development
Required Qualifications
- Active and unrestricted Physical Therapist license in the state of Colorado
- Ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
- Proficiency in medical record documentation in compliance with state, federal, and intermediary guidelines
- Commitment to patient and employee safety standards, including safe patient handling and infection control practices
- Adherence to discipline-specific professional codes of conduct and ethical standards
Preferred Experience / Skills
- Prior experience in an outpatient physical therapy setting
- Experience supervising or mentoring physical therapy students
- Familiarity with performance improvement methodologies and quality initiatives
