Key Responsibilities

Provide direct physical therapy services to patients in an outpatient clinical setting
Evaluate, assess, and develop individualized treatment plans based on patient needs
Deliver therapeutic interventions and monitor patient progress toward goals
Complete required clinical documentation accurately and in a timely manner
Collaborate with interdisciplinary team members to support patient care outcomes

Required Qualifications

Active Physical Therapist license in the state of Arizona (permanent license required)
Prior experience in an outpatient physical therapy setting
Ability to meet facility-specific credentialing and documentation requirements prior to assignment start
Must have been separated from any prior employment at this facility for at least one year to be eligible

Preferred Experience / Skills

Experience working in a high-volume outpatient rehabilitation environment
Familiarity with contract assignment workflows and travel therapy expectations
