Job Title

Speech Language Pathologist (SLP)

Location

Milpitas, CA

Schedule / Job Type

Contract (13 Weeks) | Full-Time | Days | Monday through Friday | Home Health

Key Responsibilities

Provide speech-language pathology services to patients in a home health setting
Evaluate and treat patients with communication, swallowing, and related disorders
Develop and implement individualized treatment plans based on patient needs and goals
Document patient progress and maintain accurate clinical records in accordance with regulatory standards
Collaborate with interdisciplinary care teams to coordinate patient care

Required Qualifications

Current and valid Speech-Language Pathologist (SLP) license in the state of California
Master's degree or higher in Speech-Language Pathology
Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA
Experience providing speech-language pathology services in a clinical or home health environment

Preferred Experience / Skills

Prior home health experience strongly preferred
Familiarity with home health documentation and compliance requirements
