Key Responsibilities

• Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
• Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
• Delegate and direct discipline-specific therapy services for assigned patients, ensuring care is provided under valid physician orders and within insurance approvals
• Design functional treatment programs that support effective return to community activities and progress patients toward discharge within an established length of stay
• Serve as an educator for patients, families, students, and coworkers; participate in student programs as assigned, providing supervision, learning objectives, and student evaluations
• Adhere to discipline-specific State Practice Act and all regulatory requirements; maintain professional licensure in good standing and complete all state-mandated continuing education requirements

Required Qualifications

• Active Physical Therapist license in good standing in the state of Florida
• Ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
• Proficiency in clinical documentation including goal writing, progress notes, and letters of medical necessity
• Knowledge of and adherence to applicable State Practice Acts and professional codes of conduct
• Commitment to patient and employee safety standards and safe patient handling practices

Preferred Experience / Skills

• Prior experience in an outpatient physical therapy setting
• Experience with student supervision and clinical education programs
• Familiarity with performance improvement methodologies and organizational quality initiatives
