Speech Language Pathologist (SLP) – Home Health
Location
Milpitas, CA
Schedule / Job Type
Contract (13 Weeks) | Full-Time | Days | Monday – Friday | Onsite / Field-Based
Key Responsibilities
- Provide speech-language pathology services to patients in a home health setting
- Evaluate and assess patients' speech, language, cognitive-communication, and swallowing disorders
- Develop and implement individualized treatment plans based on patient needs and goals
- Monitor and document patient progress, adjusting treatment plans as appropriate
- Collaborate with interdisciplinary care team members to coordinate patient care
- Educate patients and caregivers on therapeutic techniques and home exercise programs
Required Qualifications
- Master's degree or higher in Speech-Language Pathology
- Current and valid state licensure as a Speech-Language Pathologist in California
- Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA
- Experience providing speech-language pathology services in a clinical or home health environment
- Valid driver's license and reliable transportation for home visits
Preferred Experience / Skills
- Prior experience in a home health setting
- Familiarity with home health documentation and electronic medical record systems
- Strong interpersonal and communication skills for working with patients and families in a home environment
