Key Responsibilities

Evaluate and treat patients in a skilled nursing facility (SNF) and subacute rehabilitation setting
Develop and implement individualized physical therapy treatment plans for geriatric patients
Monitor and document patient progress toward established therapy goals
Collaborate with interdisciplinary care teams to coordinate patient care
Educate patients and caregivers on therapeutic exercises and home programs

Required Qualifications

Active Physical Therapist (PT) license in the state of Florida
Graduate of an accredited Physical Therapy program
Current BLS/CPR certification
Ability to work effectively in a SNF or subacute rehabilitation environment

Preferred Experience / Skills

Prior experience in a SNF or subacute rehabilitation setting
Experience working with geriatric patient populations
