Key Responsibilities

• Provide direct physical therapy services to patients in an outpatient clinical setting
• Evaluate, assess, and develop individualized treatment plans based on patient needs and goals
• Implement therapeutic interventions and monitor patient progress throughout the course of treatment
• Complete required clinical documentation accurately and in a timely manner in accordance with facility standards
• Collaborate with interdisciplinary team members to ensure continuity and quality of patient care

Required Qualifications

• Active Physical Therapist license required; permanent Arizona state license required (compact license may be accepted at start but permanent licensure must be obtained promptly)
• Prior experience in an outpatient or relevant physical therapy setting
• Ability to meet contract credentialing and quality assurance requirements
• Proof of license application must be provided within 5 business days of booking if permanent license is pending
• Candidates who have worked at this facility within the past year in a permanent or PRN capacity are not eligible for this assignment

Preferred Experience / Skills

• Previous travel therapy experience in an outpatient physical therapy environment
• Strong documentation skills and familiarity with electronic medical records systems
