Contract | Days | 13 Weeks | Two days 10:00 AM–7:00 PM and three days 7:00 AM–4:00 PM or 8:00 AM–5:00 PM
Key Responsibilities
- Completes comprehensive evaluations of assigned patients and develops, implements, and modifies individualized treatment plans in accordance with patient needs, goals, and physician direction
- Completes medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
- Delegates and directs all discipline-specific therapy services for assigned patients, ensuring design of functional treatment programs that support effective return to community activities
- Participates in student programs as assigned, providing supervision, developing learning objectives, and completing student evaluations
- Adheres to discipline-specific State Practice Act and all applicable regulatory agency requirements; maintains professional licensure in good standing and fulfills all state-mandated continuing education requirements
- Promotes patient and employee safety through safe patient handling, materials handling, infection control, and adherence to all clinic safety policies and procedures
Required Qualifications
- Active Physical Therapist license in good standing (Georgia)
- Minimum of 1 year of clinical experience as a Physical Therapist
- Ability to complete documentation and goal writing in compliance with state, federal, and payer guidelines
- Commitment to maintaining professional standards and adhering to discipline-specific codes of conduct
Preferred Experience / Skills
- Experience in an outpatient therapy setting
- Experience supervising or mentoring Physical Therapy students
- Active involvement in continuing education and professional development activities
