Key Responsibilities

• Completes comprehensive evaluations of assigned patients and develops, implements, and modifies individualized treatment plans in accordance with patient needs, goals, and physician direction
• Completes medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
• Delegates and directs all discipline-specific therapy services for assigned patients, ensuring functional treatment programs support effective return to community activities
• Serves as an educator for patients, families, students, and coworkers; participates in student programs by providing supervision, developing learning objectives, and completing student evaluations
• Adheres to discipline-specific State Practice Act and all regulatory agency requirements; maintains professional licensure in good standing and fulfills all state-mandated continuing education requirements
• Promotes patient and employee safety across all key areas including safe patient handling, infectious disease protocols, and fall prevention

Required Qualifications

• Active Physical Therapist license in good standing in the state of Florida
• Minimum of 1 year of clinical experience as a Physical Therapist
• Demonstrated ability to develop and implement individualized treatment plans with functional, discharge-focused goals
• Knowledge of and adherence to discipline-specific professional code of conduct and applicable state practice acts

Preferred Experience / Skills

• Experience in an outpatient therapy setting
• Familiarity with performance improvement methodologies and quality initiatives
• Experience supervising or mentoring therapy students
