Contract | Travel | Days | 8:00 AM – 4:30 PM | 13-Week Assignment

Key Responsibilities

• Provide case management services across an acute care hospital setting with a patient ratio of approximately 25:1
• Perform complex discharge planning for a diverse patient population
• Collaborate with RN Case Managers and Social Work Case Managers to coordinate patient care and transitions
• Utilize Meditech and Midas EMR systems to document and manage patient cases
• Float to applicable units as needed, including psych and rehab departments
• Work every other weekend and holidays in rotation with permanent hospital staff per facility policy

Required Qualifications

• Active Registered Nurse (RN) license valid in the state of Florida
• Minimum 1 year of experience in case management
• Meditech EMR experience required
• Midas experience required

Preferred Experience / Skills

• Experience with complex discharge planning in an acute care environment
• Familiarity with psych and/or rehabilitation patient populations
• Ability to adapt to floating across multiple units within a hospital system
