Key Responsibilities

• Evaluate and treat patients in accordance with established physical therapy standards and care plans
• Develop and implement individualized treatment programs to maximize patient outcomes
• Document patient progress and maintain accurate clinical records
• Collaborate with interdisciplinary team members to coordinate patient care
• Maintain and present valid state or federal issued photo identification at the facility at all times

Required Qualifications

• Active Physical Therapist (PT) license in the state of New Jersey
• Graduate of an accredited Physical Therapy program
• Minimum 35-hour weekly availability guaranteed
• Must wear a photo badge and carry valid state or federal issued identification while on site

Preferred Experience / Skills

• Prior experience in a skilled nursing or long-term care setting
• Experience with geriatric patient populations
• Strong documentation and communication skills
