Contract | Full-Time | Days | Monday–Friday with one weekend rotation per month | Onsite/Field-Based

Key Responsibilities

• Prioritize and complete start of care visits with next-day mandatory reporting
• Supervise Physical Therapy Assistants (PTAs) within the assigned territory
• Complete clinical notes and all required documentation within 48 hours of each visit
• Utilize the Kinnser documentation system for all clinical records
• Maintain timely communication with supervisor, intake, and office staff during normal business hours
• Meet a productivity requirement of 30 points per week, with caseloads and visit mixes expected to average 8 hours per day and 40 hours per week

Required Qualifications

• Active California Physical Therapist (PT) License required
• Minimum of one (1) year of home health experience required
• Ability to work Monday–Friday with one weekend rotation per month
• Willingness to expand territory coverage as needed

Preferred Experience / Skills

• Experience with OASIS documentation and home health visit types including start of care, recertification, and resumption of care
• Familiarity with the Kinnser electronic documentation platform
• Strong time management skills with the ability to manage a high-volume caseload independently
