Contract | Full-Time | Days | Monday–Friday with one weekend per month rotation | Onsite/Field-Based
Key Responsibilities
- Prioritize and conduct start of care visits with next-day mandatory reporting
- Supervise Occupational Therapy Assistants (OTAs) within the assigned territory
- Complete clinical notes and all required documentation within 48 hours of each visit
- Maintain timely communication with supervisor, intake team, and office staff during normal business hours
- Serve patients throughout Alameda County, with the expectation to expand territory as needed
- Meet a productivity requirement of 30 points per week, with caseloads and visit mixes expected to average 8 hours per day and 40 hours per week
Required Qualifications
- Active California Occupational Therapist (OT) License
- Minimum of one year of Home Health experience
- Ability to work Monday–Friday with one weekend per month rotation
- Proficiency or willingness to learn the Kinnser documentation system
- Overtime requires pre-approval by the Clinical Supervisor
Preferred Experience / Skills
- Experience with OASIS documentation including Start of Care, Recertification, Resumption of Care, and Discharge assessments
- Familiarity with point-based productivity models in a home health setting
- Strong time management skills with the ability to manage a varied daily caseload independently
