Key Responsibilities

• Evaluate patients and develop individualized physical therapy treatment plans
• Deliver skilled physical therapy interventions in accordance with physician orders and facility protocols
• Monitor and document patient progress, adjusting treatment plans as clinically appropriate
• Collaborate with interdisciplinary care team members to support patient goals and outcomes
• Maintain accurate and timely clinical documentation in compliance with regulatory standards

Required Qualifications

• Active Physical Therapist (PT) license in the state of Nebraska or eligibility for licensure
• Graduate of an accredited Physical Therapy program
• Must reside at least 80 miles from the facility to be eligible for standard contract rates
• No on-call or call-back availability required

Preferred Experience / Skills

• Prior experience in a skilled nursing or long-term care setting
• Familiarity with geriatric patient populations and restorative therapy approaches
• Strong communication and interdisciplinary teamwork skills
