Key Responsibilities

• Provide skilled physical therapy services to a geriatric home health patient population
• Conduct and document Start of Care assessments using OASIS
• Manage a caseload of up to 6 patient visits per day, maintaining 30 productivity points as the standard
• Utilize HCHB EMR for all clinical documentation
• Travel independently to patient homes within the service area, with occasional drives of 50+ miles

Required Qualifications

• Active California Physical Therapist (PT) license required
• Basic Cardiac Life Support (BCLS) certification required
• Minimum of 1 year of recent skilled home health experience (at least 1 year within the last 3 years)
• OASIS experience required
• Proficiency with HCHB EMR required
• Must have reliable personal transportation

Preferred Experience / Skills

• Experience working with geriatric populations in a home health setting
• Strong time management skills with the ability to efficiently manage a multi-patient daily caseload
