Key Responsibilities

• Coordinate care for patients on a structural heart unit, serving as a primary point of contact for care transitions
• Perform administrative duties including managing referrals and patient follow-ups
• Conduct triage as required within the care coordination role
• Utilize Epic EMR system for documentation and care management tasks
• Collaborate with interdisciplinary teams to support patient care plans with less direct patient-facing interaction than a typical RN role

Required Qualifications

• Active Registered Nurse (RN) license
• Minimum 2 years of nursing experience required
• Level 1 trauma experience required
• Triage experience required
• BLS certification through the American Heart Association (AHA) required
• Epic EMR experience required
• Proficient computer skills required, including experience with Microsoft Excel
• Must reside more than 50 miles from the facility to be eligible

Preferred Experience / Skills

• Background in structural heart or cardiac nursing
• Prior experience in a care coordination or administrative nursing role
