Description

The Hospice Nurse admits patients to service, assesses the patient’s and family’s needs, and develops an individualized care plan. This role involves coordinating care with a multidisciplinary team and participating in quality improvement initiatives. Follow-up visits are conducted as necessary across the designated territory.
